Client Rights

Effective August 2026

As a client of 3:16 Behavioral Health & Wellness, PLLC, you have the following rights throughout your care.

To be treated with dignity, courtesy, and respect, free from discrimination or harassment.
To receive services without regard to race, color, national origin, language, religion, age, sex, sexual orientation, gender identity, disability, marital status, or veteran status.
To know your counselor's name, credentials, license status, and the nature of the services offered.
To give informed consent, to ask questions, and to participate in setting your treatment goals.
To confidentiality within the limits of the law, and to be told what those limits are before you share.
To refuse any recommended service, to end services at any time, and to be informed of the likely consequences of doing so.
To request a referral to another provider or a second opinion.
To choose whether faith is included in your counseling; faith-based content is always optional and never required.
To know fees in advance, including cancellation and no-show policies, and to request a Good Faith Estimate.
To access your record and request corrections as described in our Notice of Privacy Practices.
To be free from any form of exploitation, including sexual, financial, or personal.
To voice a concern or complaint without retaliation, and to file a complaint with the appropriate state licensing board.
To request interpretation services or reasonable accommodations for a disability.

Your responsibilities include arriving on time, giving accurate information, following the agreed cancellation policy, and telling your counselor when something is not working so care can be adjusted.
